The admins at the place should have presented a DNR if there was one, but they didn't, so you had to talk to the family. Their emotions and ignorance led to things getting fucked up. Luckily this wasn't the case for the doctors and nurses so they had it easier to do the right thing. Old lady got her wish, though not how she wanted; but family got their wish by seeing someone care about saving her and trying to do something about it. You did your job and you did it well.
